We are located on the corner of Giles and Wentworth St at 1B/61 Giles St, Kingston. There is free parking around us on the street. When you visit for the first time we kindly ask that you arrive 10 minutes earlier to fill out a new patient form and COVID screening questionnaire. These details will include your general details as well as a detailed medical history form. If you have any current medications or medical issues please bring in your medication list or any relevant information from your doctor or specialist.
